Documents You’ll Need

The required paperwork for an Arkansas disability placard varies depending on your specific situation:

For New Permanent or Temporary Placard Application:

  • You’ll need to provide Form 10-366 with medical certification completed and signed by a licensed medical professional.
  • Valid Arkansas driver’s license, state ID card, or Social Security number required for identification.
  • Replacement for lost or stolen placards requires new Form 10-366 with current medical certification and valid identification.
  • Disabled veterans have an alternative process through your local Veterans Administration office. Provide proper VA documentation of your service-related disability instead of standard medical certification.
  • Organizations providing transportation for persons with disabilities may apply for special permits. Submit certification Form 10-336 completed by an authorized official and marked with disability code F.
  • The organization must provide their Federal Employer ID number instead of social security number or driver’s license.
  • Van-accessible parking requires separate decal in addition to regular placard with certification for transporting wheelchair, scooter, or walker users.
  • Disabled motorcycle plates available through Special License Unit only using same criteria as passenger car plates.

  • If you’re applying through a telehealth service like ParkingMD, you can get the evaluation done completely online, You’ll receive your completed Form 10-366 via email, typically within 24-48 hours, ready for DMV submission

Replacement for Lost or Stolen Placards:

  • Complete a new Form 10-366 with current medical certification
  • Provide valid identification
  • Submit your application at any Arkansas Office of Motor Vehicles

Permanent Placards

  • Expire every 4 years and must be renewed before the expiration date.

Replacing a Lost, Stolen, or Damaged Permit

  • For temporary permits: replacement is only possible if the original permit hasn’t expired; the replacement will have the same expiration date as the original.
  • Present paperwork at any Arkansas Motor Vehicle office. There’s no fee for permanent or temporary replacements.
  • You must remove and return permits when selling your vehicle or when eligibility ends.
