Make sure that all of your information matches your current driver’s license. I myself had forgot to add my middle initial and other information, so I was originally rejected by my local DMV. Once corrected, I returned to the DMV and all was good.

Just make sure you have all your proof from your doctor’s to upload. whether they are x-rays MRI scans or notes from your doctor about your conditions showing that it came from the doctor and it is about you. how will your dogs in a row and everything will be fine.

I followed the instructions and connected with the doctor very easily and uploaded my medical records and very shortly thereafter it was approved for my parking permit. This was so easy to follow the instructions and get my parking permit.
