Depending on your situation (new application, replacement, or renewal), you will need some or all of the following:
New Permanent or Temporary Placard
- Form HSMV 83039 completed and signed by an approved medical provider within the last 12 months.
- A valid Florida driver’s license or state-issued ID card.
- The $15 fee when applying for a temporary permit.
- Applications may be submitted in person or by mail to a motor vehicle service center.
Veteran Exemption
- Veterans certified as permanently and totally disabled due to a service-connected disability may submit VA Form Letter 27-333, or its equivalent.
- The letter must have been issued within the last 12 months and may replace the medical certification
Online Doctor’s Certification
If using a telehealth service like ParkingMD:
- You’ll receive a signed medical certification on Form HSMV 83039 by email within 24–48 hours.
- You can print and submit it to one of these Duval County motor vehicle service centers:

ParkingMD provides the medical evaluation and certification only. A Duval County motor vehicle service center reviews the application and issues the physical permit in Jacksonville.
Lost or Stolen Placard Replacement
- Complete Form HSMV 83146.
- Provide Form HSMV 83039 signed within the last 12 months when required.
- Submit the documents to a motor vehicle service center.
- Bring a copy of your current permit registration, if available.
Note
- If your disability prevents you from visiting an office to obtain an ID, your physician may complete the exemption section on Form HSMV 83146.
- Permanent permits are valid for four years and have no state fee.
- Temporary permits are valid for up to six months and require a $15 fee.
